In consideration of the agreement of Ohio Rural Electric Cooperatives, Inc. (“OREC”) and
to participate in an all-expense paid trip to and from Washington, D.C., coordinated by OREC, from June 14-20,2026] (the “Youth Tour”), I, the undersigned, being the parent(s) or legal guardian(s) of the Student, do hereby consent that the Student may participate in the Youth Tour.
I acknowledge and agree that the Student will travel by bus from Columbus, Ohio, on June 14, 2026, and return to Columbus, Ohio, on June 20, 2026, and that chaperones from OREC and the Cooperative will accompany the group on the Youth Tour.
I have read and understand this Assumption of Risk, Release, and Waiver Agreement (the “Agreement”) in its entirety. I understand that participation in Youth Tour activities is entirely voluntary and requires participants to follow all instructions and abide by all applicable rules and standards of conduct. I understand that participation in Youth Tour involves risk of personal injury due to the physical, mental, and emotional challenges related to the activities involved. I represent that Student has no known mental or physical conditions that would impair Student’s abilities to participate in Youth Tour.
I further request and authorize OREC acting through its chaperones to secure any medical or other emergency services the said chaperones in their reasonable discretion may believe to be necessary or desirable for the Student during the Youth Tour. Without limiting the generality of the foregoing, in the event of bodily injury to or illness of the Student at any time during the Youth Tour, if I or another emergency contact provided cannot be immediately contacted, the undersigned grants full power of attorney to OREC acting through its chaperones to do as follows:
- To arrange for the transportation of the Student, whether by ambulance or otherwise to a proper facility where emergency medical treatment would normally be administered, including but not limited to, an emergency room of a hospital, a doctor’s office, or a medical clinic; and
- To sign any release as may be required in order to obtain any medical or surgical treatment as is required in the judgment of medical authorities at the facility.
I assume all risks, known and unknown, foreseeable and unforeseeable, in any way connected with Student’s participation in Youth Tour. I accept personal responsibility for any liability, injury, loss, or damage in any way connected with Student’s participation in Youth Tour. I understand that by participating in Youth Tour, Student may be exposed to injury, the injury of others, or injury to property due to Student’s activities, the activity of others (including, but not limited to the National Rural Electric Cooperative Association (“NRECA”), OREC, and the Cooperative) or conditions related to Youth Tour activities. I have carefully considered the risks associated with Youth Tour and hereby give my informed consent for Student to participate in all Youth Tour activities.
IN CONSIDERATION OF THE OPPORTUNITY AFFORDED TO STUDENT THROUGH PARTICIPATION IN YOUTH TOUR, AND WITH APPRECIATION OF THE RISKS ASSOCIATED WITH YOUTH TOUR ACTIVITIES, ON MY OWN BEHALF AND ON BEHALF OF STUDENT, TO THE FULLEST EXTENT PERMITTED BY LAW I HEREBY FULLY AND COMPLETELY RELEASE OREC, THE COOPERATIVE, AND THEIR RESPECTIVE DIRECTORS, OFFICERS, EMPLOYEES, CONTRACTORS, AGENTS, VOLUNTEERS, CHAPERONES, SUCCESSORS, AND ASSIGNS FROM ANY AND ALL LIABILITY FOR AND WAIVE ANY AND ALL CLAIMS FOR ALL LIABILITY, CLAIMS, CAUSES OF ACTION, DEMANDS, COSTS, OR EXPENSES OF ANY KIND OR NATURE, WHETHER BASED IN CONTRACT, TORT, STATUTE, OR OTHER LEGAL OR EQUITABLE THEORY OF RECOVERY, IN ANY WAY CONNECTED WITH STUDENT’S PARTICIPATION IN YOUTH TOUR, INCLUDING, BUT NOT LIMITED TO, ANY LIABILITY ARISING OUT OF OREC’S EFFORTS TO OBTAIN EMERGENCY MEDICAL TREATMENT ON STUDENT’S BEHALF, WHETHER OR NOT CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OR OTHER MISCONDUCT OF OREC, THE COOPERATIVE, OR THEIR DIRECTORS, OFFICERS, EMPLOYEES, CONTRACTORS, AGENTS, VOLUNTEERS, CHAPERONES, SUCCESSORS, AND ASSIGNS.
I understand and agree that the above release and waiver is material consideration for Student’s participation in Youth Tour. I further understand and agree that by signing this Agreement I am giving up substantial legal rights. I understand and agree that this Agreement shall be legally binding upon me, my relatives, personal representatives, heirs, beneficiaries, next of kin, or assigns and shall inure to the benefit of OREC, the Cooperative, and their successors and assigns.
